Risk Management
For the protection of all soccer players and volunteers, Indiana Youth Soccer has
established a Risk Management Program which includes the use of
employment/disclosure statements for all coaches, managers, club board members and
anyone else in a club having significant contact with the players or with club
funds. Beginning in the Fall of 2005 all volunteers MUST fill out a disclosure form
on line and have a background check run. The procedure is outlined below.

The disclosure form is located on the Indiana Youth Soccer website,
www.indianayouthsoccer.org under the Risk Management heading.

All adults needing background checks must access this form, fill it out and submit
it online.  This only needs to be done once during the season.  If you filled it out
in the Fall you DO NOT need to fill it out for Spring.  

Please note: Your personal information will remain confidential. The Club will only
be notified in the event that a disqualifying offense is discovered during the
background check process.
